Medical Consent Form
Participant Release Form Committee Request Form

 

Early Bird Registration: Payment received by March 14th - $190.00 per family, $330 for family with 2 daughters.

Registration: Payment received by June 1st - $200 per family, $ 350 for family with 2 daughters

Late Registration: Payment received by September 10th - $ 250 per family, no discount for 2 daughters

 

WE WILL NOT ACCEPT REGISTRATIONS AFTER SEPTEMBER 10TH.

IN ORDER FOR YOUR REGISTRATION TO BE CONSIDERED COMPLETE WE NEED EACH

FORM COMPLETELY FILLED IN WITH YOUR CHECK MADE OUT TO EK KARDIA, INC.
